Various form of gastritis have been depicted in the below :
Type A :
(a) Atrophy of parietal cell mass hence hypochlorhydia & decrease intrinsic factor .
(b)Antrum is not affected .
(c) Autoimmune - antibodies to parietal cell .
(d) Hypochlorhydria stimulates antral gastrin .
Type B : (a) H.pylori associated .
(b) Antrum is affected .
(c) predisposes to peptic ulcer disease .
(d) Can give rise to intestinal metaplasia , dysplasia & for the development of malignant disease.
Reflux : (a) After cholecystectomy ,gastric surgery .
(b) Antrum is affected .
(c) usually does not give rise to peptic ulcer disease .
(d) Treated by prokinetic agents or bile chelating agents .
Erosive : (a) It occurs due to NSAID. (b) It affects entire stomach . (c) It occurs due to defective gastric mucosal barrier . (d) Treated by H2 blockers .
Type B gastritis & erosive gastritis is common .Endoscopy & Biopsy are the key investigations in all forms of gastritis .The treatment depends upon the type of gastritis .Rarer forms of gastritis include granulomatous gastritis , lymphocytic gastritis etc .
